Pre-Op Care Package Request
Who is a care package for? Our Pre-op Care Packages are designed to suit those who are undergoing breast reconstruction surgeries (mastectomy, DIEP Flap, LAT Flap). Recipients will need to be currently scheduled for their surgery due to either a diagnosis of breast cancer or at high risk of developing breast cancer (additional phases of surgery do not qualify). The pre-op care package recipient will need to reside in the state where the package is being sent to.
***We are currently only licensed in MI. We are unable to accept donations outside of our state at this time.***

As a Medical Professional, how can I request care packages for my practice?
If you are a doctor, or work for a doctor’s office who performs these surgeries in the state of Michigan, and would like to be able to have these packages available to distribute to your patients please fill out the form below. We will contact you to make arrangements to have them delivered.
**Please note that the pre-op care package availability is entirely dependent on funding. We purchase in bulk as funding allows, so there may be times when packages are not available.
